A13 YEAR OLD FEMALE COME TO THE CHIEF COMPLAINT OF BLOOD IN URINE AND SWELLING OF THE BODY

CASE STUDY
Fever since 1 month
Blood in urine since 1 month
HOPI : Pt was apparently asymptomatic 1 months ago then she developed fever low grade intermittent type relieved with medication
Not associated with chills and rigor
Vomiting which is non bilious contain small food particles 4 episodes/ day
Loose stools 3 to 4 episodes/Day
Not associated with pain
Now patient presented to the OPD with the complient of hematuria which is red in colour since 1 month
No burning micturition
Decreased urine out put
Then she developed initially both lower limb of pedal edema then gradually seen in face puffy ness of edema
Edema in abdomen
No history of sore throat infection
PAST HISTORY
No similar history in the past
No history of hypertension ,epilepsy, bronchial asthama ,DM, TB
FAMILY HISTORY.
No history
MENSTRUAL HISTORY
Age of menarche at 11 year
Lmp 25/11/22
PERSONAL HISTORY
Loss of appetite
Diet mixed
Adequate sleep
Bowel and bladder movement normal
No history of allergy
GENERAL EXAMINATION
Patient was concious coherent cooperative
Examined in sitting position
Paller++
No icterus
No cyanosis
No clubbing of finger s
No lymphadenopathy
Pedal edema present pitting type of edema

Afebrial on touch
Temperature 98.4F
