A  45 Year old female patient come with      swelling in the lower both legs                                                       Hi, I'm soniya 3' rd sem medical student   .this is an online elog bookto discuss our patients health data after taking his consent this is also reflects my pt centared online learning portfolio   
                                                                          A 45 year old female . She is resident of thallakampa . Labour by occupation.came to hospital.  . C C: swelling in the both legs   Associated with pain                                                                   HoPI: pt was apparently asymptomatic 2 years ago
She she presented with pain 6 months ago.later .then she developed gradual pitting type of edema  in  both lower   legs since 20 days. pain is there when pressure applied. 
She developed HTN since 20 days 
4  days  back left leg edema is gradually decreased ( normal)
Present right leg  edema is  there (unilateral)   
She doesn't have any fevar 
Noo chest pain     

  Past history:   No DM  No asthama. No epilepsy.                              
HTN  present since 20 days      
                                                             Personal history : 
 Diet: mixed.                                      Appetite: normal.                                    Sleep: adequate.                                    Bowel movement: regular.                         No addiction s.      
    
 . Family history: no Family history.        Allergic history: no history of allergy.      

 General examination  pt was c/c/ c.                 Moderetally nourished
     Moderetally built                                     No pallar .lcterus. cyanosis. No clubbing fingers. No lymphadenopathy.       

    Vitals : bp 120/80 mm of hg                                        PR 86 BPM                                                        RR 18 cpm.                                                      Temparature: 98,6'F.    
     Systemic examination                                                 CVS:  S1 ,S2heard no murmers.                    CNS : no focal neurological defects.                                                                             Rs: no added sounds.                    Investigation:                                                    .  RBS( random blood sugar)
